Services alone do not make a person feel at
home, that is why at SJNH we make special
effort to provide services and programs
that enhance the quality of daily living.

Services and programs offered are based on the individual person's needs and strengths. In order to develop an individual plan of care that meets the medical needs of your loved one, we will need a copy of his/her medical history including the most recent physical examination. Services and programs include:

Skilled Nursing: 24-hour nursing care. Our nurses are available to residents and their families to explain and answer any questions regarding resident’s care plan.
Restorative Therapy: Daily exercises and individualized program for restorative nursing care.
Recreational & Social Activities – Encouraging all to lead full lives, ongoing activities are designed to meet the interests and the physical, mental and psychosocial well-being of each person living here.
Social Services: Helping residents adjust to a new living environment, assisting them and their families with referrals, assessments, and community networking.
Rehabilitation Services – Under the direction of their personal physician: Speech, occupational and physical therapy on site available to residents with impaired functional skills.
Resident/Family Support Groups: Monthly meetings are held to address family concerns with follow up plan of action.
Staff Development: Active participation of staff in identifying the challenges to personal growth that can improve the overall care provided to our residents.
Religious/Pastoral Services: Religious/pastoral services are provided to accommodate resident’s preferences. All are welcome to participate in services provided. In room services/visits available to residents who are bedridden.
Quality Assurance Program: Committees work to identify potential and or actual problems and to bring about the best possible solution.
Dietary Services: Individualized meal planning provided by registered dietician based on your loved one’s dietary needs and personal preferences.
