From the Hospital: The discharge planner or Social Service Department is the usual liaison between the hospital and the nursing facility. The Social Service Department is available to assist in implementing placement procedures.

From Home: Once the primary physician has recommended long term care, contact our Social Service Department for assistance in the admission requirements. The following medical information will be needed: Medical History, Physical Exam, & Physician Orders.

From Another Facility: If you plan to transfer - speak to the Social Service department/Admissions Coordinator at the present facility for assistance in making the necessary adjustments - documentations to facilitate transfer.

       Upon admission, client will need an attending physician. Please check with current physician to assure that he/she is willing to continue to provide medical care once client is living in a nursing home. If new physician needs to be selected, we will be happy to assist you in selecting one.
A designated responsible party must be confirmed on admission. This person is to assist in the admission process and help make subsequent decisions about his/her loved one. To make the day of admission easier for everyone, we suggest that an appointment be done prior to admission. At this appointment, responsible party should bring the following documents pertaining to his/her loved one:

• Social Security card
• Medicare card
• Insurance card(s)
• Medicaid card
• Guardianship papers for Health Care
• Durable Power of Attorney
• Living Will

THINGS TO BRING:
For your loved one’s comfort we suggest a seven to ten day supply of comfortable, machine washable clothing. The following clothing items are recommended (as applicable):

• dresses
• pants
• shirts/tops
• sweat suits
• socks/stockings
• underwear
• pajamas
• bathrobe
• shoes (non-skid soles
• slippers
• sweaters
• coat
• hats/gloves
